D.Margaretha Lundin has her expertise in social work and passion in improving the health and wellbeing. She has built this patient education together with her team which includes a doctor, a registered nurse, an urotherapist, a dietician, a dental hygenist, a hospital librarian, an occupational therapist, an enrolled nurse, a physical therapist and a speech therapist. During the process, a group of COPD patients, their relatives and The Swedish Heart and Lung Association were also involved in the process
Abstract:
Introduction : COPD will be the third leading cause of death worldwide by 2030. Hospital strategies to improve care for patients are pivotal. Evidence shows that patients with COPD can improve their health and minimize hospital visits through self-management education. However, not every patient with COPD are able or offered to participate in patient education. Purpose : To reach more patients with COPD and their relatives. Methods : The COPD-team is a large and unique team, which includes 11 different health professionals and together with COPD patients we have reviewed and transformed the older more traditional education model to be more patient centered. Result : The cooperation with the COPD patients led to a website containing text, images and 28 videos. This website enables patients and their relatives to participate in self-management education whenever and wherever they wish.
